Title: Comparing the evaluation of right ventricular systolic function in individuals with and without idiopathic right bundle branch block

Abstract

Introduction: Right bundle branch block (RBBB) is one of the most common electrocardiographic abnormalities that may lead to poor outcomes in patients with cardiovascular diseases. Previous studies reported the impact of idiopathic RBBB on several right ventricular systolic function variables. Thus, the necessity of evaluating RBBB patients is more felt. This study aimed to compare the evaluation of the right ventricular (RV) systolic function in individuals with and without idiopathic right bundle branch block.

Materials and Methods: In this comparative cross-sectional study, 52 patients with idiopathic RBBB and 60 normal individuals were enrolled. The variables examined in this study were demographic information (age and gender) and echocardiographic findings such as strain (longitudinal strain), strain rate, right ventricular fractional area change (FAC), left ventricular ejection fraction (LVEF), and mitral inflow.

Results: The global longitudinal strain, strain rate, and time to peak (except the E wave index in the lateral wall) of the whole right ventricle and the septal and lateral wall separately were significant (P <0.01). The mean and standard deviation of the FAC index was statistically insignificant. The LVEF index in both groups was in the normal range (LVEF<50%) and was statistically insignificant. Mitral inflow was statistically insignificant. Statistical analysis of the data was insignificant in terms of gender. 
 
Conclusion: Subtle abnormalities can be seen in RV systolic function in patients with idiopathic RBBB. Thus, this individual group feels more need for regular clinical evaluation.

Amir Savarrakhsh graduated as a cardiovascular specialist at the age of 30 from Guilan University of Medical Sciences. Throughout his university years, he was always a diligent and interested student. Therefore, he was the top student for consecutive years. Immediately after graduating, he became a faculty member at Guilan University of Medical Sciences. Dr. Savarrakhsh has 8 publications over 3 years of working and his publication h-index is 1.
