Title: Lactate as a marker of the effectiveness and safety of physical rehabilitation in “inotrope-dependent” patients with chronic heart failure, who are candidates for heart transplantation

Abstract

Purpose: To assess the lactate content in the central venous blood and its changes in response to physical activity (PA) in patients with chronic heart failure (CHF) receiving treatment with dopamine or dobutamine, and to analyze adverse events (AEs) during participation in the physical rehabilitation program (PRP).

Materials and methods: Prospective randomized follow-up for 6 months included: men 18-65 years old, with coronary heart disease or dilated cardiomyopathy; terminal CHF III-IV functional class, LVEF ≤30%. 80 patients receiving dobutamine or dopamine≥ 2 weeks were randomized into 2 groups: 1st group – participation in the PRP, 2nd group – not participating in the PRP. In the 3rd group 40 patients participating in the PRP without the need for inotropic therapy were included.

Сonclusions: Against the background of participation in the PRP in 1 gr. patients, there was a tendency towards a decrease in lactate, while in group 2, not participating in the PRP, there was a tendency towards its increase. By the 6th month of the study, blood lactate became lower in the groups of patients who participated in the PRP compared to those who did not participate, which confirms the effectiveness of this PRP. Peak blood lactate values were not accompanied by clinically significant AEs and indicated its safety.
