An unusual presentation of Takotsubo Syndrome in a young female of middle eastern origin
Abstract
A young girl with no known comorbidities presented with chest pain, palpitations, and an ECG showing ST depression along with positive troponin, suggesting non STEMI , Echocardiography revealed inferior and basal segment hypokinesia, an ejection fraction of 50%, and a Global Longitudinal Strain (GLS) of -14, indicating myocardial dysfunction. CT coronary angiography ruled out congenital coronary artery anomalies, and cardiac MRI confirmed the echocardiographic findings, showing mild edema in the inferior wall but no scar tissue (normal Late Gadolinium Enhancement or LGE). These findings pointed toward a transient heart condition rather than a typical ACS.The diagnosis of Takotsubo Cardiomyopathy (TCM), or “broken heart syndrome,” was made. TCM is typically triggered by stress and mimics a heart attack but often shows full recovery. The patient was started on bisoprolol 2.5 mg daily, a beta-blocker aimed at managing heart function. At her follow-up four weeks later, her heart function had significantly improved, with normal left ventricular wall motion and a GLS improved to -21.This rare presentation of TCM in a young girl, outside the typical older female demographic, suggests the need for genetic testing to explore potential underlying genetic factors contributing to the unusual age of onset. Moreover, the improvement in GLS serves as an important prognostic marker for recovery in TCM. The significant GLS improvement correlates with the complete resolution of myocardial dysfunction, indicating that GLS could be a reliable guide in predicting recovery outcomes in Takotsubo Cardiomyopathy. Further studies may provide deeper insights into the genetic basis and prognostic factors for this condition.
