Symptom control in individuals with advanced heart failure
Abstract
Introduction : Heart Failure (HF) is a frequent outcome of cardiovascular diseases, characterized by a worse prognosis, the appearance of multiple symptoms, and potential mortality of the individual, requiring early initiation of Palliative Care (PC). Nursing is the main link between patients, family, and members of the multi-professional team, being in a strategic position for the assessment, approach, and intervention of symptom control in individuals with progressive disease and gradual worsening, such as advanced-stage Heart Failure.
Objective : To systematize and evaluate the levels of evidence and degrees of recommendation on the most effective nursing interventions for symptom control in PC patients with advanced heart failure.
Methodology : Systematic review (SR), structured with PRISMA, with an evaluation of the level of evidence and OCEBM (Oxford Centre for Evidence-based Medicine levels of evidence), degree of bias with the Cochrane Handbook, Scimago quality assessment, and critical reading with CASPe.
Results : A total of 4197 articles were obtained, of which 15 documents were included in the review; 11 randomized controlled clinical studies (A1b) and four systematic reviews (A1a), with Q1 quality, except one with Q2, with an average compliance with the CASPe critical reading tool of 96%.
Conclusion : The most effective interventions in symptom control in individuals with advanced HF in PC are educational-behavioral, pharmacological, spiritual, and end-of-life, where the nurse plays a crucial role in their care.
