Title: The Prevalence of Familial Hypercholesterolemia among Adult Filipino Patients in a Tertiary Hospital in Bicol, Philippines

Abstract

Background: Familial hypercholesterolemia is an autosomal dominant disorder with elevated low-density lipoprotein cholesterol (LDL-C) levels. The estimated prevalence worldwide is 1 in 500. However in Asia the latest guidelines claimed no true estimate forits prevalence. 

Methods: An observational descriptive prospective study involving Filipino patients, age 19 years and older, from Camarines Sur, with dyslipidemia, done at Universidad de Santa Isabel Health Services Department. The sample size was computed using the Slovin’s Method. The formula used was the total number of population in Camarines Sur divided by one plus total population multiplied by the margin of error squared. Confidence level was assigned at 95% and a margin of error at 0.05. The computed sample size was 399. Those who fulfilled the inclusion criteria were assessed using the Dutch Lipid Network Criteria (DLNC) for the diagnosis of familial hypercholesterolemia. 

Results: One hundred twenty five patients were collected during the initial two-month data-gathering period. Eighty-three subjects were female, and forty-two were males. Age ranges between 22 to 96 year old. Three patients were excluded from the study due to their address coming from Camarines Norte and Albay. From the remaining one hundred twenty three patients, 42 (34%) scored unlikely, 31 (25%) scored with probable, 36 (30%) scored with possible, and 13 (11%) scored with definite familial hypercholesterolemia.

Discussion: In the preliminary result of this study, 13 (11%) of the study population fulfilled the criteria of definite familial hypercholesterolemia. This is a very high prevalence rate when compared to what is claimed worldwide, which is only 0.025%. This result should raise the awareness of our healthcare providers with regards to early detection and management of familial hypercholesterolemia especially those with high likelihood or at high risk for inheriting the disease.
