Title: Mitral valve prolapse arrhythmogenic syndrome

Abstract

There is an association between mitral valve prolapse (MVP) and sudden cardiac death. It is prudent to identify and stratify patient at risk. Among the risk factors were female gender, electrocardiographic abnormalities, presence of myocardial fibrosis seen by magnetic resonance imaging and mitral annulus disjunction diagnosed by echocardiogram and/or magnetic resonance imaging. Most patients who develop sudden death associated with MVP have evidence of a “trigger” (premature ventricular contractions) associated with a substrate (myocardial deformation or fibrosis secondary to mitral annulus disjunction) necessary to initiate and perpetuate malignant ventricular arrhythmias. Future longitudinal studies are needed to advance our understanding of the mechanisms involved in sudden death associated with MVP, to validate existing risk factors, and to identify high-risk patients who could potentially benefit from intervention for primary prevention of sudden death.
