Comparison of De Vaga Versus Ring Annuloplasty in Patients with Moderate to Severe Tricuspid Regurgitation

Abstract

Background: This study compares the outcomes of tricuspid regurgitation (TR) using the De Vaga technique versus ring annuloplasty in terms of achieving adequate pulmonary artery pressure (PAP) and functional outcomes.
Methods: A comparative study included 100 adult patients who underwent mitral valve surgery with tricuspid repair. They were divided into two groups: Group A (n=50) underwent mitral valve replacement (MVR) and TR repair using the De Vaga technique, while Group B (n=50) had MVR with tricuspid repair using ring annuloplasty. The De Vaga technique involved placing 2 pledgeted parallel running 4-0 polypropylene stitches at 3-4 mm intervals in a semicircular manner from the postero-septal to the antero-lateral commissure, with adjustment using a valve gauge based on body surface area.
Results: Females predominated in the study. The mean surgery duration was 66.4 minutes in Group A and 71.0 minutes in Group B, which was not statistically significant. Length of stay in the ICU and overall hospital stay were similar between both groups (p=0.68). There was no statistically significant difference in postoperative functional capacity distribution (PAP) between the surgical techniques (p=0.19).
Conclusion: The De Vaga repair technique is comparable in durability to prosthetic ring annuloplasty, with additional benefits of being a simpler, cost-effective, and time-saving approach.
