Application of ECG in diagnosing and evaluating therapeutic changes throughout the entire course of hypertension
Abstract
Objective: To explore the significance of electrocardiogram (ECG) in diagnosing hypertension and evaluating therapeutic outcomes.
Methods: A retrospective analysis of ECGs and treatment improvements in hypertensive patients was conducted to summarize characteristic patterns and staging.
Results:
1.Hypertension induces three major ECG abnormalities: left atrial/ventricular hypertrophy, myocardial ischemia, and arrhythmias.
2. ECG can classify hypertension into four progressive stages: pre-early, early, intermediate, and late stages, each with distinct features:
Pre-early stage: Left atrial enlargement (prolonged P-wave duration ≥0.12s), mild myocardial ischemia (up-sloping ST-segment depression ≥0.05mV, flattened T waves), with gender differences (prolonged P-waves more common in males; ST-T changes more frequent in females).
Early stage: Left atrial enlargement (prolonged P-wave duration ≥0.12s, abnormal Ptfv1, bifid P waves), mild myocardial ischemia (horizontal ST-segment depression), and occasional arrhythmias (sporadic atrial premature beats).
Intermediate stage: Left atrial enlargement (as above), left ventricular high voltage (Rv4/Rv5/Rv6 ≥2.5mV), moderate myocardial ischemia (horizontal or down-sloping ST-segment depression with shallow T-wave inversion), and arrhythmias (various premature beats or tachycardia).
Late stage: A left atrial/ventricular hypertrophy (prolonged P-wave duration ≥0.12s, abnormal Ptfv1, bifid P waves, left QRS axis deviation, left ventricular hypertrophy with repolarization abnormalities), severe myocardial ischemia (horizontal or down-sloping ST-segment depression with deep T-wave inversion), and severe arrhythmias (significant tachyarrhythmias or bradyarrhythmias).
3. ECG can assess improvements in hypertension treatment.
Conclusion: ECG reflect pathological changes throughout the entire course of hypertension and provide a valuable tool for evaluating therapeutic efficacy.
Biography
Liantao NIE completed master's education in 2016, engaged in electrocardiogram diagnosis and research, and led/completed 2 municipal level scientific research projects; Received 1 first prize for scientific and technological achievements from the Education Department of Henan Province and 3 national utility model patents; Participated in the compilation of 5 monographs on electrocardiograms, published over 10 professional and popular science papers, updated over 300 popular science short videos, and was invited to participate in the World Congress on Cardiology (Mexico) in 2016 and give a poster presentation. Won the first prize in the 2017 China Electrocardiogram Good Case Award, the third prize in the 2019 China Huaxia Electrocardiogram Competition, the first prize in the 2020 Zhengzhou Electrocardiogram Skills Competition, the Zhengzhou Technical Model, the excellent popular science worker in Henan Province's fight against the epidemic in 2021, and the first prize in the 2023 China Electrocardiogram Society Network Public Welfare Classroom "Libang Cup National Electrocardiogram Competition.
