Invasive Candida Discitis and Endocarditis in a Patient with an ICD and Multimorbidity: A Conservative Management Challenge
Abstract
This case report details the complex medical journey of a 61-year-old male with multiple comorbidities, including emphysema, heart failure, ischemic heart disease, and a history of ventricular tachycardia managed with an implantable cardioverter defibrillator (ICD). He presented with weight loss, poor appetite, and malaise, leading to a diagnosis of acute spondylodiscitis and Candida bloodstream infection. His management involved multiple adjustments in antibiotic and antifungal treatments due to recurrent infections and complications, including tricuspid valve vegetation and recurrent ICD shocks. Despite successful nutritional support through a nasojejunal (NJ) feeding tube, his overall prognosis remained poor due to frailty and unsuitability for surgical intervention for fungal endocarditis. This case underscores the importance of a multidisciplinary approach and highlights the limitations of conservative management for invasive Candida infections. It emphasizes the need for prompt, tailored treatment strategies to improve outcomes in patients with severe comorbidities and compromised health.

Zeyad Khalil completed hist MBChB at the age of 24 years from Manchester University. Currently training in internal medicine in the North West of England. He has numerous publications including in Nutrients and British Society of Cardiology (BSC). He hopes to continue training into cardiology with a subspeciality in electrophysiology and cardiac devices.
