Balloon pulmonary angioplasty for inoperable CTEPH: Early outcomes of clinical implementation in a hospital without on-site cardiothoracic surgical backup

Abstract

A total of 109 BPA procedures were performed in 38 patients. The mean age was 64.4 ± 14.2 years. Mean pulmonary artery pressure decreased by 15.3% (Δ –8.1 mmHg, p < 0.001), systolic pulmonary artery pressure decreased by 11.1% (Δ –10.6 mmHg, p < 0.001). The 6 MWD increased by an average of 13.1% (+50.6 m, p < 0.001). The proportion of patients with NYHA I/II increased from 13.5% to 32.4% (p < 0.001). NT-proBNP decreased by 48% (p = 0.005). The number of PAH medications and their combinations did not change significantly (p = 0.083). Regarding complications, perforation of pulmonary artery branches occurred in 4% of cases (5 perforations out of 109 interventions), and hemoptysis occurred in 2.8%. No cases of grade 3–4 reperfusion edema, intubation with mechanical ventilation, ECMO, or in-hospital mort

Biography

Vakhtang Safaryan received his doctoral degree in 2008. Over the following years, he completed a residency in internal medicine and a fellowship in general and interventional cardiology. He currently works in the Department of Interventional Cardiology at Moscow City Hospital No. 29 and conducts outpatient consultations at the affiliated polyclinic.
